چکیده

has led to an appreciation of the clinical importance of achieving regression of cardiac hypertrophy, as well as control of blood pressure, in patients receiving antihypertensive therapy. The superiority of converting enzymeinhibitors compared to other antihypertensive agents in effecting regression of cardiac hypertrophy could be attributable to their characteristic profile of actions. By inhibiting the production of A II, they not only decrease afterload through blood pressure reduction, but also ameliorate myocardial ischemia through improvement in myocardial blood flow resulting from increased myocardial and arteriolar compliance due to reduction in the size of individual myocardial cells and regression of interstitial and perivascular tissue flbrosis. Recent findings that converting enzyme inhibitors improve the prognosis for patients with heart failure can be readily understood based on the underlying pathological events leading to a hypertrophic heart.

Brain mechanisms of sympathetic activation in heart failure: Roles of the renin‑angiotensin system, nitric oxide and pro‑inflammatory cytokines (Review).

Patients with chronic heart failure (CHF) have an insufficient perfusion to the peripheral tissues due to decreased cardiac output. The compensatory mechanisms are triggered even prior to the occurrence of clinical symptoms, which include activation of the sympathetic nervous system (SNS) and other neurohumoral factors. However, the long‑term activation of the SNS contributes to progressive car...

Evaluation of intravenous injection of amniotic membrane stem cells to reduce inflammatory factors in heart failure

Background: Heart failure is one of the most common cardiovascular disorders and is considered a chronic, progressive and debilitating disorder. The medical treatment of this disease is accompanied by many problems. Today, stem cells are being used increasingly to reduce the problems of heart failure treatments. Since pro-inflammatory cytokines play an important role in the prognosis and progre...

Progression of heart failure after myocardial infarction in the rat.

This study examined the early neurohumoral events in the progression of congestive heart failure (CHF) after myocardial infarction (MI) in rats. Immediately after MI was induced by coronary artery ligation, rats had severely depressed left ventricular systolic function and increased left ventricular end-diastolic volume (LVEDV). Both left ventricular function and the neurohumoral indicators of ...

Cellular and molecular aspects of contractile dysfunction in heart failure.

A number of molecular and cellular alterations have been identified in the failing human heart that help to understand contraction and relaxation abnormalities. Cyclic AMP dependent pathways are desensitized due to quantitative changes in beta-adrenoceptors, beta-adrenoceptor kinase, and inhibitory G-proteins. Calcium homeostasis is impaired, characterized by a decreased calcium reuptake rate o...

Metabolic mechanisms in heart failure.

Although neurohumoral antagonism has successfully reduced heart failure morbidity and mortality, the residual disability and death rate remains unacceptably high. Though abnormalities of myocardial metabolism are associated with heart failure, recent data suggest that heart failure may itself promote metabolic changes such as insulin resistance, in part through neurohumoral activation. A detrim...

The predictive roles of self-efficacy, illness perception, and social support in self-care of patients with heart failure

Background: As a chronic disease, heart failure (HF) is a growing problem that not only impacts patients but also their family members and friends. Self-care is the main part of HF treatment. Despite the significance of determining variables involved in self-care, the relationships of self-efficacy, illness perception, and social support with self-care and the predictive roles of these variable...
